Dorchester, Kent, Queen Anne's and Talbot County Health Departments are seeking organizations interested in delivering the CDC National Diabetes Prevention Program (DPP) to communities within the mid-shore region. Four mini-grants of $7,000 each will be awarded to organizations who commit to delivering the DPP to one cohort of at least 15 people.
Existing grantees need not apply.

Prediabetes
Diabetes is the seventh leading cause of death in Maryland. Approximately 11.2% of adults in Maryland have been told they have diabetes and another 1.8 million adults in Maryland have prediabetes, a condition where blood sugar levels are higher than normal, but not high enough to be diagnosed as type 2 diabetes.
Most people with prediabetes do not know they have it, putting them at an increased risk for developing type 2 diabetes, heart disease, and/or stroke. Individuals with diabetes are twice as likely to have hypertension, further increasing their risk for heart disease and stroke. In addition, 89.8% of adults with diabetes are overweight/obese and 19.8% of adults with diabetes are current tobacco users. Having multiple comorbidities, such as diabetes, hypertension, obesity, and tobacco use, increase the risk of diabetes related complications.
The CDC National Diabetes Prevention Program (DPP)
The National DPP is a structured, evidence-based, year-long lifestyle change program proven to prevent or delay the onset of type 2 diabetes in adults with prediabetes or at risk of developing type 2 diabetes. The National DPP utilizes lifestyle coaches trained to deliver a CDC-approved curriculum and have the knowledge and skills to effectively deliver the program with fidelity to the current Diabetes Prevention and Recognition Program (DPRP) Standards. Participants meet once per week for the first six months, and then monthly for the last six months.
DPP within the Mid-Shore Region
Since 2022, the mid-shore counties have been working together to expand the CDC National Diabetes Prevention Program (DPP). The Eastern Shore Area Health Education Center serves as the region's DPP hub, offering a virtual program and working with community based organizations in each county to offer in-person programming. With funding from the Maryland Department of Health, local health departments are seeking to expand program offerings to include more in-person options in the following counties: Dorchester, Kent, and Queen Anne's and Talbot.
Eligible Applicants:
Businesses, non-profits, faith-based and other community-based organizations that serve residents of Dorchester, Kent, Queen Anne's or Talbot counties are eligible to apply. Organizations will need to submit a W-9 in order to receive payment and must be in good standing with the State of MD.
